Health Care Systems Essay

Abstract The primary objective of any health care system is to provide adequate and effective medical care to the population. Health care systems may vary due to political and other factors. Factors may include location, access to care, basic needs of the populations as well as economic status. However, the primary goal remains the same. Because of the ongoing need for government to allocate funds to the medical budget their involvement in health care cannot be discounted. This paper introduces two major health care systems. First that of the Netherlands and secondly, the United States. The pros and cons will be discussed, as well as the role and function of the government as it relates to health care. A compare and contrast of the differences and similarities of both systems will be made. Health Care Systems The Netherlands The health care system in the Netherlands is comprised of three distinct compartments and is mandatory for all residents and non-resident who pay Dutch income tax. They are required to purchase health insurance coverage, except for those with conscious objections and active members of the armed forces. Coverage is mandatory under the health insurance act provided by private insurance companies and regulated under private law. One percent of the Dutch population were uninsured in 2009 and approximately sixteen percent between the ages of twenty and thirty years. Those who failed to pay premiums for at least six months are also known as defaulters. (Westert & Klazinga, 2011, p. 1) Insurance companies are forbidden to perform â€Å"risk assessment† that deny coverage based on pre-existing conditions, risk factors based on age, gender, or health profile. Tax credits make the package affordable for those who have low income while those who have no income receive coverage as part of their social assistance package. (Daley & Gubb, 2011) The government provides health care allowances also known as premium subsidies for low-income families if their premium exceeds five percent of the household income. (Westert & Klazinga, 2011, p. 80) Individuals who do not sign up for health care coverage are subject to a tax fine of one hundred and thirty percent of the premium. (Daley & Gubb, 2011) The Exceptional Medical Expenses Act regulates the first compartment. (Daley & Gubb, 2011) Contributions were taken as a 12.55% salary deduction and further supplemented by a government grant. (Daley & Gubb, 2011) Basic insurance packages include General Practitioner, Hospital specialists, Midwives, as well as hospitalization, dental coverage up to the age of eighteen after which only specialist dental care is covered (dentures e.g); medical aides such as home health care and medical equipment pharmaceutical care, maternity, ambulance and patient transport services, paramedical care, mental health and limited lifestyle improvement (smoking cessation programs e.g.). In vetro fertilization is also covered for the first three attempts. (Westert & Klazinga, 2011, p. 1-2) The second compartment deals with basic and essential needs. These medical needs are first treated by a General Practitioner. Every resident and non-resident must be registered with a General Practitioner who oversees basic care including, physicals and common illnesses. In the event specialized care is needed, patients are referred to a Nurse Specialist who is responsible for giving medical treatment. This may include information pertaining to prevention, education, social and psychological support. (â€Å"Nursing,† n.d) For example if a patient is diagnosed with Diabetes Mellitus their first encounter is with the General Practitioner who makes the diagnosis, and puts together a team consisting of the Diabetic Nurse Specialist and an Endocrinologist. The Endocrinologist supervises the Nurse Specialist who in turn acts as a supervisor or consultant to the General Practitioner. Members of the team must meet on a regular basis to discuss the patient’s needs and progress. Based on the severity of the patient’s illness (which could be deemed as low, moderate or severe) determined by the patients’ test results one of the three care givers will in fact manage follow-up care. However, all three practitioners will continue to be involved. The Nurse Specialist acts a liaison between both the Practitioner and the Specialist. (â€Å"Nursing,† n.d) The third is an optional supplemental coverage and is paid for by the individual or as part of an employer/employee contract. Care can include, dental after the age of 18 years, physiotherapy and cosmetic procedures. The focus on overcoming disparities such as obesity and smoking cessation do not exist in the Netherlands, as it is believed that it is the responsibility of the individual to seek treatment, which is covered under the basic package. Before the 2006 Reform, there were long waiting lines to see the General Practitioner. There are forty Dutch health insurers across the country and individuals are free to choose the company of their choice based on their personal needs and preferences. At the time of annual open enrollment, patients are allowed to change their insurance company to one of their own choosing. (Westert & Klazinga, 2011, p. 84) Every insured individual age 18 and over must pay a deductible ranging from â‚ ¬170 to â‚ ¬ 670 referred to as cost sharing. General practitioner care and children health care are exempt from cost sharing. (Westert & Klazinga, 2011, p. 80) How is the health system financed? – The first â‚ ¬ 32,369 are taxed at a rate of 6.9 %. The employer is required to reimburse this contribution while the employee must pay taxes on the reimbursement. For those who do not have an employer or do not receive unemployment benefits the contribution is 4.8% while the self-employed is individually assessed by the Department of Revenue. (Westert & Klazinga, 2011, p. 80) Organization – The General Practitioner is the considered to be the gate- keeper. The General Practitioner must refer hospitalization or specialized health care and the only exception is for emergency care. The General Practitioner gets a capitation fee for each registered patient and is further compensated for after hours care on an hourly basis. Consultation fees including phone consults are also accrued and an additional amount is paid to the General Practitioner for managing the patients care without having to make a referral. Bundled payments are made for chronic diseases such as Diabetes Mellitus, Chronic Obstructive Pulmonary disease, Congestive Heart Failure etc. If the General Practitioner hires a private nurse to assist in his practice, the insurance company makes full reimbursement to the General Practitioner for nurses’ salary. The population of the Netherlands is 16.7 million people (â€Å"One World Nations Online†) of which only one percent is uninsured. The reason for this is not due to an inability to pay but rather a default for greater than six months. The health care system in the Netherlands is Universal and does not depend on employment status. (Westert & Klazinga, 2011, p. 78) Although the government mandates heath insurance, private insurance companies are allowed to provide coverage. With five insurance companies that dominate the market the government has created a market environment for healthy competition that also benefits the consumer. (Westert & Klazinga, 2011, p. 80) The triangle between the Insurer, the Provider and the Insured requires that quality and efficient care remains consistent. There are five non-governmental entities that regulate care. The Health Council advises the government on health care issues (e.g. public health); The Health Insurance Board (advises what should be included in the basic health insurance packet). The Medical Evaluation Board is responsible for efficiency, safety and quality are always taken into account; The Dutch Health Care sAuthority, which ensures that the market is functioning while the Dutch Competition Authority ensures there, is fair competition among insurers and providers. (Westert & Klazinga, 2011, p. 83) The United States In the United States government is heavily intertwined with health care at many different levels. Laws have been created by both the federal and state legislative bodies. Health care policies have been shaped through a combination of the arenas that make decisions at the local, state and federal levels. The United States health care system is comprised of two separate entities identified as the private and public sectors. First, the private sector is made up of private health insurance companies and employer sponsored insurance coverage. Beginning in 2014, the Affordable Care Act of 2010 will require employers who have fifty or more employees to provide health insurance coverage or pay a financial penalty to the federal government. Medicaid program will be expanding and will be required to cover those who fall above the level of poverty.. (Kaovner & Knicman, 2011, p. 36) This like the health care system in the Netherlands tries to cover a greater amount of the population The public s ector is made up of Medicaid and Medicare. Medicaid is a welfare initiative that is not a single national program, but a collection of fifty state-administered programs. (Kaovner & Knicman, 2011, p. 29) Medicaid is designed to cover low-income families. It is jointly funded by the state and federal government. The poorer the state the larger the federal contribution however, each state has specific eligibility rules, benefits and payment schedules. A little known fact about Medicaid is that it can be given free to those who cannot afford it, or it can be given at a low cost depending on one’s income. (Social Security†) Medicare on the other hand is a social as well as a federal insurance program designed to cover people aged 65 and older as well as the disabled regardless of age or income. There are four parts to Medicare. Part A also known as hospital insurance covers inpatient hospital stays, care in a skilled nursing facilities, hospice and some home health care. Part B also known as medical insurance covers certain doctor’s services, outpatient care, medical supplies and preventative services. Part C also known as the medical advantage plan is a type of medical health care plan offered by a private company that contracts with Medicare to provide Part A and Part B benefits. Part D, also known as the prescription drug coverage, can be added onto the original Medicare plan. The aforementioned plans, are offered by insurance companies and other private companies approved by Medicare. (medicare.gov). The public sector is designed to cover the segment of the population that is not covered by employer-sponsored insurance. The United States highest annual health care spending per capita in terms of purchasing power parity is $7,538 is significantly higher than the Netherlands, which spends $4, 063 per capita. This trend has remained constant for the past forty years. (Kaovner & Knicman, 2011, p. 68) It is reported that the United States has the largest proportion of adults who have the hardest time seeing a specialist, however other countries have reported the same difficulties with low-income levels having a greater difficulty than those with higher income. (Kaovner & Knicman, 2011, p. 69) The life expectancy in the United States for male was 75.64 years while women was 80.78 years. In the Netherlands on the other hand, life expectancy for a male is 81.4 years and for female 85.3 years a significant difference and is believed to be due to the quality and access to health care. The quality of life in the Netherlands is assumed to be much better because of preventative care and education. Patient Protection and Affordable Care Act of 2010, children in the United States will now be covered up to the age of 26. (Kaovner & Knicman, 2011, p. 25) In the Netherlands children are only covered until the age of 18 at which time they are expected to be contributing members of society. Approximately 16% of the United States population is uninsured, which is a significant disparity between that of the Netherlands. Americans are uninsured primarily because of high out-of-pocket medical expenses and considerable amounts of medical debt. Most of those uninsured are families with full- or part-time jobs or those who are self-employed will not be able to afford or access health care. (Kaovner & Knicman, 2011, p. 32-33) Health care provisions are adequately satisfied in the Netherlands, which make provision for all Citizens while still leaving room for healthy competition among insurance carriers. Although guidelines are set up by the government to ensure universal health care it is the responsibility of insurance companies and non-governmental agencies to ensure that these guidelines are followed. On the other hand, while the United States supports those with low income and disabilities there remains the 17% of the population that contribute to the Medicaid and Medicare funds but are still unable to afford health insurance coverage. I believe it is to the detriment of the United States health care system to pull certain aspects from many different health care systems such as the Netherlands and Switzerland that do not always work to our benefit. (Kaovner & Knicman, 2011, p. 79) The absence of a body to oversee making checks and balances in the systems leaves the door open for fraud and fraudulent activities within the system. If these aspects were to be imported it must be modified and carefully monitored to ensure there is no conflict with existing programs that have been proven effective. If the average household income per year is $40,000 of which 30% is spent on heath care it leaves a family with insufficient funds to provide for basic needs. In effect, health care becomes the â€Å"basic need†, creating further health care needs due to the inability to cover food, clothing and shelter. In conclusion, it is my opinion that the health care system in the Netherlands is by far more advanced in terms of organization and effectiveness. The ability for all residents to access health care contributes to the longer life expectancy and overall quality of life which in together ensures that the people will be valuable contributors to society. Thus the distribution of the grain and butter is mainly done by the government of the country to the people of the country. The production of even the basic grain and butter is done by the factories that are under the ownership of the government. Erstwhile China as well as Cuba and North Korea are examples of such economics where the principles of communism are being practised. The government in such countries is the sole decision maker regarding the level of production and the allocation of resources (Conklin, 1991, p. 427). Alternatively, in case of the capitalist economies the process of production is undertaken by the private players. In almos t all the capitalist economies there is a government but the function of the government is restricted to the regulatory aspects of the country and to ensure that the laws are obeyed. The main function is limited to the collection of the taxes and the protection of the citizens (Stephen, 1998, pp. 31-49). The market power lies mainly with the players that are dominant in the market. The UK, the US as well as the German economies constitute the capitalist nations (Slater and Tonkiss, 2001, pp. 31-40). The characteristics of these nations are that free market is allowed to operate and the prices in the economy are determined by the forces of the demand and supply. The prices of grain and butter would be determined by the market forces of demand and supply. Thus the optimal production would take place at the point where the quantity demanded would be equal to the quantity supplied. For instance, developing specific learning goals and a plan of action is an excellent start to promoting cultural awareness. My desire to raise my cultural awareness of two specific groups, particularly homosexuals and individuals belonging to the black population, require that I set personal learning goals and a plan of action that will guide how I am going to go about the process of learning and understanding culture based on race, ethnicity, and sexual orientation. Because of my parents and family, I was able to understand that people are different from each other, and although we are different, I do not have the right to judge other people because of it. Handling the matter only requires respect and acceptance and the understanding of cultural diversity or multiculturalism. Judging from my self-assessment of my cultural awareness and competency, I believe I will be able to uphold moral and ethical principles in my profession, especially with issues that concern multiculturalism. The direct contributions which are generated from income taxes, tourism employment, businesses related with tourism such as travel agencies, public boats, marinas etc. and of course from direct taxes on the traveling tourists such as departure taxes. The second category is the indirect contributions, generated from taxes and duties taxed and products and services supplied to the tourist for example accommodation and food. Leaving Poland aside for a moment and viewing this on a worldwide scale the WTO had estimated in 1998 that direct and indirect tax contribution to economies around the globe surpassed 800 billion US $. This year (2010) the WTO estimates this figure to be double. Employment generation The rapid expansion of tourism in Poland the recent years lead to a significant increase in and creation of employment. For examples Hotels alone provide around #### jobs Furthermore tourism generates jobs also directly through restaurants, means of transport, nightclubs, and travel agencies, souvenir sales and tourists attractions while it also generates jobs indirectly through the supply of goods and services sectors associated with tourism related businesses. In total tourism supports about 437,600 workers in Poland. Stimulation of infrastructure investment Since a nice country is an attractive country, tourism motivates the government to work on improving the infrastructure of destination sites which slowly spread throughout other parts of the country as well. Such improvements among others are better water systems, sewage and waste handling systems, roads, electricity, communication means and quality, transport networks and so on. These improvements of course do not only help attract tourism but benefit the life of the local population. Contribution to local economies The environment is a basic part of the assets used by the tourism industry and for that reason profit coming for the field of tourism is measured to measure the economic value of protected areas. But local revenues are not always easy to qualify as tourism related ones that happens because not all of tourist expenses are formally registered. In Poland as in any country money is also earned through informal employment, for example from street vendors, informal guides or rickshaw drivers. The positive side of informal employment however is the fact that the money theses people earn returns to the local community as they engage their own transactions as they too have to live. This money gets a great multiplier effect as it is spent over and over again. The multiplier however is not only attributed to the informal revenues any transaction between any tourist related business and the visitors or two or more business entities which purchase items or services one from another within the local economy. What has as a result the inflow of money to Polands economy from en exterior source and this money is subsequently spent again by its new owner in a different transaction and so on. In this way the market and the different sectors is fueled extra cash. For example, the tourist will take a taxi to go to his hotel, the taxi driver will then use that money to pay for a service to his car, the mechanic will use the money he gained to go to the super market, the super market will then order new stock and so on. The multiplier is itself is split into 3 categories. Direct expenditure, is when the tourist personally exchanges money for a good or service, in the hotel or at a restaurant or at a cinema etc. It is also related with tourism produced exports and with investments related to a tourism project in an area. Indirect expenditure, is the transaction made subsequently to the one described above by the new owner of the money. For example when a hotelier pays for goods or services provided by the local area like ordering food supplies or taking extra staff. Induced expenditure is the increased purchasing activity of locals due to the additional personal income resulting from the Direct expenditure of visitors. For example, the hotel employees use their wage bonuses or tips to buy something at the super market. Induced and Indirect expenditures are also called secondary expenditure. Based on this the WTO assumes that tourism generates an indirect contribution the economy which equals to 100% of the direct tourism expenses. Economic Disadvantages of tourism in Poland. Leakage The direct income from tourism oriented transactions is the amount of money that remains after subtracting taxes, profits and wages paid to sources outside of the area and imports are bought. These amounts removed from the overall amount are leakages. Fire example in the all inclusive holiday packages more or less 80% of the tourist money goes to international companies which own airlines, tour operators, hotels etc. while the locals receive very little of that money. Additionally the remaining retained income can still leave Poland through subsequent leakage or transactions the locals do with informal or wandering vendors. Import Leakage, happens when the visitors expect to find or demand standards of equipment, food or beverages Poland does not produce or cant supply by itself. Champagne, feta cheese, bananas and natural gas are only some examples of industries or products Poland cant produce for various reasons or it simply does not yet have a supplying industry. Furthermore a significant amount of the income made from tourism leaves Poland again to pay for these imports. The average import related leakage is assumed to be between 40 to 50% of gross tourism earnings for small economies and between 10 to 20% for more advanced countries. Polands leakage on imports is estimated to be 36% Export leakage, international companies play a big role in this kind of leakage. Particularly in poor developing destinations they are the ones that have the required capital to invest on building of tourism infrastructure facilities and improvements. As a result the foreign investors who sponsored the construction of hotels and other facilities take their share from the tourism related revenue back to their own countries. Enclave tourism Takes place when the more developed countries try to control the tourism development in developing destinations or use that as an opportunity to increase their own incomes. As the international companies intervene with tourism activities of les advanced countries, local businesses see their chances to earn money from tourists greatly reduced. Since if the visitor remains for his whole stay only in the hotel which offers him all he wants (food, drink and entertainment) local market has little ways to benefit from tourism. While all inclusive packages generate big revenue little of that reaches the local economy compared to other forma of accommodation. These types of establishments also import more trying to gain cheaper prices and employ less people than normal establishments. Infrastructure costs The development of tourism costs the government and Polish taxpayers great sums of money. Order, Disorder and Imagination in John Buck’s Sculptures There is always a first impression, a first glance seeping through the corner of your eye. When passing by one can’t help but to notice the overwhelming presence of John Buck’s freestanding sculptures which stand in front of the Gallery. I find myself overwhelmed by both the size and the boldness by which these sculptures speak. Their organized structural nature combines a variety of ideas from branches, birdhouses to molecular structures and globes, and in the middle of it all stands the ringing human form. Initially, the forms and elements I saw in these sculptures impressed me but their composition did not challenge or stimulate my curiosity. So I kept up with the flow of traffic and continued on walking by. I couldn’t quite figure out why I had an aversion to these sculptures. It wasn’t until I started to break down the elements of these sculptures and take a closer look within the walls of the gallery that my first impression faded and a new aur a of his work came into being. A have found an appreciation for both his ways of making and the perceptions he presents. I am a big fan of wood carving, so I am thoroughly impressed by John Buck’s carvings of both his sculptures and woodblock prints. I appreciate the details of his work for instance the visibility of the chip carving and the control he has over the bends of the wood. I am curious to know the techniques he uses to get these details and his methods to making them. I read that John Buck uses a special kind of wood called jelutong. The artist said about working with wood, â€Å" There is an automatic and direct relationship between the nature of the wood and how I am able to work with it†¦In the carving of wood there is a physical activity that is more about nature in the making –not just the concept of the image, but the actual making, is connected to nature.† His connection to nature and the environment go beyond his use of wood as a medium, but are explicit in the stories he tells. In the lecture ‘Pillar to Pole’, Bruce Gunter commented on three forces which he found impacted Buck’s art work, these being: Midwest roots, environmental degradation and relationships to humanity.

â€Å"In Mrs. Tilscher’s Class† by Carol Ann Duffy deals with one central theme. The theme of growing up is the main idea within the poem and is repeatedly imprinted throughout the poet’s childhood. This theme leads on to the more abstract idea of the child already maturing into a great poet. Her mind’s eye is unbounded as she transforms her classroom into a place of riches and resides in her own world of imagination. Written improbably through the 2nd person viewpoint, the poem expresses these ideas personally to the reader, hence allowing us to empathise with the poet. The poet is able to recall several aspects of her primary school days, and is consequently able to paint a picture of her memories from the viewpoint of a young child. The writer not only conveys an inviting warm atmosphere of a 1960’s classroom, but also unveils a liberal outlook to her childhood. A colourful classroom with numerous displays is made known to the reader: â€Å"The classroom glowed like a sweet shop. † The classroom is made into a place of riches with this visual simile, used to radiate wonders of the child’s mind. The word â€Å"glowed† in this line is a metaphor all on its own. The metaphor allows the reader to visualise the sweetshop gleaming due to the light refracting through the glass jars and translucent sweets. The poet can also bring to mind the teacher’s blackboard, as she informs the reader of how â€Å"the chalky Pyramids rubbed into dust. † In a literal sense the chalky lines on the board became chalk dust. The poet imagines this to be great pyramids and monuments being eroded inevitably by time. The bell signifying the end of playtime is remembered as: â€Å"The laugh of a bell, swung by a running child. † This auditory image incorporates the personification of the bell, to compare its sound to an incessant laugh of a child. The bell’s laugh is a transferred epithet from the child, as the children too were laughing, overwhelmed with joy as they returned to their classroom for another dose of Mrs. Tilscher. The poet’s joy is so intense and infectious, that it reaches out and transforms the whole scene. Such is the magnitude of the poet’s emotion. The laugh is also a visual image, as the reader can see a smile as the arc of the bell, and the clapper hitting the sides of it is almost comparable to a tongue. However other images such as a â€Å"skittle of milk† are more informative and suggest the time setting of the poem. The poet also remembers a music room next door to her classroom, though only by means of â€Å"a xylophone’s nonsense† heard. This auditory image describes the noise of the xylophone next door and this is further expanded by the use of the single metaphoric word â€Å"nonsense† which implies the vague unclear noise heard and the fact that the primary school children are producing uncoordinated music. The â€Å"enthralling books† were not to be forgotten to the poet, as they had made her a slave to them continually, due to their alluring influence. All the images used to recall aspects of the poet’s primary school principally focus on an emotional and sensual level. The primary school classroom may have been a place memorable to the poet through various images, but the definitive piece of the poet’s 1960’s school life was Mrs. Tilscher. Mrs. Tilscher’s voice is not even forgotten, as the poet reminisces her voice as she â€Å"chanted the scenery. † Mrs. Tilscher’s chanting brings about connotations of music in her voice and melodic speech. It also brings about a sense of religion, as she is made comparable to a pastor in a church chanting a sermon, enlightening and entrancing us all. Mrs. Tilscher is portrayed as a compassionate teacher: â€Å"Mrs. Tilscher loved you† and shows kindness and care. The terms around the lines focused on Mrs. Tilscher also have an implication on how she is illustrated to the reader. Words with intense connotations such as glowed, sweet, sugar and coloured have associations with moods of joy, brilliance, love and bliss which all elaborate on Mrs. Tilscher’s image. The teacher is also illustrated to be appreciative: â€Å"Some mornings you found she’d left a good gold star by your name. † Although it seems that the poet finds aspects of the classroom just as unforgettable as Mrs. Tilscher, the poet essentially portrays the classroom’s essence to be the work of Mrs. Tilscher, through her tone of voice. Choices of words or diction such as â€Å"could† have connotations of possibility. When put into context and further developed on, the implications go as far as unconstrained and limitless possibility. The teacher opens up a whole world of possibility, and it is because of this the poet remembers so much about her classroom, a boundless environment. It is because of Mrs. Tilscher that the poets classroom surroundings were made to be so memorable. The poet’s tone of voice and language varies throughout the poem, and strong distinction is made between the first two stanzas and the last two stanzas. The language in the first two stanzas is exceptionally exuberant, more child-like in an emotional sense and the imagery is much more pleasant to envisage, filled with colour, vibrancy and liveliness: â€Å"Sugar paper. Coloured shapes. † Each individual phrase builds up an atmosphere full of warmth. However the last two stanzas are less joyous in their atmosphere, as the poet makes her transition to a state of being overwhelmed by hormones. The word connotations also vary greatly in these two stanzas, bringing suggestions of anger, accusation and dismay: â€Å"You kicked him, but stared at your parents, appalled. † The stanzas are also bleaker in description and imagery is uninviting: â€Å"The air tasted of electricity. † The poet’s feelings in stanza four are troubled, after being introduced to topic of how she was born: â€Å"A tangible alarm made you always untidy, hot, fractious under the heavy, sexy sky. † Such sentences furthermore use more mature and sophisticated language, which coincide with her growth as the language also develops. Therefore the poet’s outlook and tone of voice changes as she becomes much more interested in growing up than going to primary and learning in a high-spirited vibrant classroom. The reader of the poem is invited to personally explore the main ideas within the poem. This personal involvement of the reader seems appropriate, as the main theme conveyed, the journey of growing up, is your own personal exclusive journey. This personal involvement is due to the narration of the poem from the second-person viewpoint. This is shown through the excessive use of the second-person pronoun â€Å"You. † By using this narration style, the experience of the young poet is made universal and common. We can all be subjected to her experiences of growing up from the second-person narrative perspective. Although the reader finds it easy enough to face the poet’s experiences, Mrs. Tilscher feels that she should have no influence in the young child’s journey of growing up, and that such a journey should proceed at the person’s own pace. When the child asks the teacher about how she was born, â€Å"Mrs. Tilscher smiled, then turned away. † Mrs. Tilscher may have believed that the poet would learn in her own time, but the poet nevertheless encourages the reader to enter her journey. The poem illustrates two worlds in which the poet resided during her childhood days. The reader is able to capture not only the essence of the classroom, but also the limitless realm of the child’s imagination. Both these worlds exist alongside each other agreeably as the classroom is made into a creative place itself due to the influence of Mrs.Tilscher. The foremost apparent world presented by Carol Ann Duffy is the classroom. The classroom conveys images of riches, sweets, colour and joy. However beyond this, the classroom is seen to be a sanctuary. The classroom was a safe house against the world of murder and crime outside, as suggested by the mention of â€Å"Brady and Hindley† of the 1960’s. The real world begins to force an entry into the child’s reverie as she slowly begins to become conscious of the world outside. This is the first occasion in which the poet shows signs of growing up, which enforces the main theme of the poem. The girl learns that the real world isn’t to be trusted. The classroom however is portrayed as a world of its own, not troubled by the likes of such horrific murderers. The fear fades away in the classroom, and along with this so does the little hint of adulthood. She postpones her transition into adulthood for the meantime, as the poet shows us by using a child-like image after the allusion: â€Å"Brady and Hindley faded, like the faint uneasy smudge of a mistake. † This second component to the sentence indirectly illustrates the use of a pencil, and the occurrence of â€Å"silly mistakes. † It is because of this implication that the poet moves back into the state of childhood. The growth of the poet is exemplified in this classroom world, and therefore this world is very significant to the theme. Conversely, on a more abstract plane, the poem portrays another world within the child’s mind. A whole world of imagination and vision. The poet expresses that she â€Å"could travel up the Blue Nile with your finger tracing the route. † The poet is tracing her finger down the Nile, and is in her imaginative world of Egypt. On the other hand, in reality the teacher is demonstrating sketch maps on the board. The word within the poem, which establishes both worlds within and without, is a metaphor on its own. The word â€Å"travel† is the single metaphor, which suggests that the poet is on a journey within her mind, when actually she is stationary within her school seat. The poet also conveys the main theme in this imaginative world, as the child not only travels with her finger and during her daydream, but she furthermore travels through her journey of growing up. The child is beginning to grow into a great poet, and this is shown through various lines within the poem. A very strong contributor to this idea of the girl growing into a great poet is an example of synaesthesia: â€Å"The scent of a pencil, slowly carefully shaven. † This image appeals to all of the senses at once, and incorporates kinetic, olfactory, visual and tactile aspects. This line shows how the poet â€Å"carefully shaved† her pencil, just in the same way she carefully crafts sentences. This is extended even further by the link made between the writing tool and the writing process. The child poet is even able to link this image full of senses to the main theme of growing up. The act of the girl carefully shaving the pencil, symbolises how she is shaving or peeling off her childhood as she makes the transition into adulthood. The poet gradually conveys to the reader that there are two states of growth within the poem, and that the girl is maturing both into adolescence and into a mind of an exceptional poet. The last stanza of the poem focuses the atmosphere and the attitude of the poet into an uninviting overcast, but also centers in on the theme. It illustrates the â€Å"feverish† month of July, oppressed by the summer and heat. Alongside this are the hormones of the child, felt almost within the air. These hormones amplify the effect of the afflicting heat. The air also â€Å"tasted of electricity†, which conveys the anticipation of summer thunderstorms due to the heavy air. However â€Å"electricity† also relates with the hormones to suggest that the child will spark at random times and also that the growth of the child is full of charge, energy and excitement. Further along, the use of the phrase â€Å"a tangible alarm† portrays an almost touchable fear within the air. This fear made the girl â€Å"fractious under the sexy sky. † This expresses to the reader that the girl had many sudden outbursts of anger due to her hormones. These hormones influence her thoughts and are the causes behind the poet using the term â€Å"sexy† to describe the sky. The last line of the poem communicates how this adolescent phase is like a thunderstorm. The thunderstorm represents her feelings of puberty, as she feels as though the whole world is coming down on her, just as in a thunderstorm. The lightning of a thunderstorm also links to the connotations of the â€Å"electricity. † The lightning of the thunderstorm could symbolize the mood swings awaiting the child. The lightning also illustrates the fact that there is an unsettlement within the child, as if an electric current was continually running through her. The rain of a thunderstorm conveys the downpour of gloom upon the child throughout the hard times to come. On an overall view the experience of puberty and growing up is just a phase and in time will pass. Soon the child will be entirely in adulthood. Likewise the thunderstorm is just an unpleasant phase in the sequence of weather and in time shall pass. Before long the sun will overpower such a ghastly occurrence of weather. Overall, â€Å"In Mrs.Tilscher’s Class† by Carol Ann Duffy is a poem which allows the reader to personally identify themselves with the poet. The poem is contrastive between the stanzas and thus the poet is able to isolate the main idea. Two worlds are created expressing the wonders of the classroom, but also illustrating the unconstrained world of the girl’s imagination. Through these two worlds we see signs of the girl growing into a great poet. However the most essential idea of the poem is the theme of growing up and maturing.
